This closes long-standing <a href="https://github.com/nedbat/coveragepy/issues/310?featured_on=pythonbytes">issue 310</a> as well as its duplicates: <a href="https://github.com/nedbat/coveragepy/issues/312?featured_on=pythonbytes">issue 312</a>, <a href="https://github.com/nedbat/coveragepy/issues/1673?featured_on=pythonbytes">issue 1673</a>, <a href="https://github.com/nedbat/coveragepy/issues/1845?featured_on=pythonbytes">issue 1845</a>, and <a href="https://github.com/nedbat/coveragepy/issues/1941?featured_on=pythonbytes">issue 1941</a>.</li>
